Then complete, sign and scan the On Air Release and email to our Sweepstakes administrator at Live@enteractivesolutions.com or you have the option to upload your signed and scanned On Air Release form on the Thank You page once you've successfully submitted your Online Entry.
Your first name, last name and the email address you used in your Internet Entry must be inserted into the email subject line.
Once we have your On Air Release, we will keep it for subsequent Internet Entries, so please email just one!

The snapshot should be an amateur morning wake up themed photograph of the entrant only, must not exceed 150 KB and must be in one of the following formats: .JPG, .JPEG or .GIF format.

When directed by you to do so through completion of the form below, the photo you upload may be used, without limitation, in an online photograph gallery (which may include a user voting feature with respect to your photo and others), in on-air broadcast(s) or for marketing or promotion of the website or program.
